The Second Schedule decides most Indian applications

The SMC registers international medical graduates whose basic medical degree comes from a university or medical school listed in the Second Schedule of the Medical Registration Act 1997, or who hold a registrable postgraduate qualification recognised by the SMC. Its registration page is explicit that graduates whose qualifications are not recognised in either way “will not be eligible to apply”.

In the version of the Second Schedule incorporating S 857/2024 (in force from 1 January 2025), India has two entries:

A graduate of any other Indian medical college therefore needs a registrable postgraduate qualification recognised by the SMC. The SMC publishes that list as a separate document, linked from its registration pages.

What conditional registration requires

According to the SMC, the applicant must:

• hold a registrable basic or postgraduate qualification, as above
• have been selected for employment in an SMC-approved healthcare institution
• hold a certificate of experience, or equivalent, showing satisfactory completion of housemanship, PGY1 or the internship year
• have been in active clinical practice for the three years before applying, at an average of at least 168 hours a year
• have passed the national licensing examination of the country where the basic degree was obtained, if that country requires one
• be certified to be in good standing by the overseas regulatory body
• meet the SMC’s English language requirements, if the basic qualification was not taught in English

Working under supervision

All conditionally registered doctors practise under a supervisor approved by the SMC. Their clinical work may not include aesthetic practice, and health screening should not exceed 20% of their caseload. The SMC describes progression through levels of supervision; a doctor may join private practice after reaching Level 3 (L3), subject to specialty-specific experience thresholds.
